SBPA Membership Roster

Please complete this form at the start of each season to ensure that all of our info is up-to-date and you don’t miss important communications about SBPA or Shawnee Band activities.

For each student, please tell us:

• Name, incoming grade, and position in the band
• Parents’ names, emails, and phone numbers
• T-shirt size (for their free tour shirt) and shoe size (for new members)
• Any dietary restrictions, allergies, or other concerns
